If you’re a Truli for Health member, you can also access your current SBC anytime by logging in to the Truli for Health member portal. 

Once you click on the link below, use the Search box to search for an SBC by Plan Number. If you are unable to locate your SBC or wish to have an SBC sent to you free of charge, call 855-30TRULI (87854). For TTY/TDD call 1-800-955-8771.
